The first step
will be to complete the Pledge Form on this page. The information
you provide will ensure proper handling of your donation. The second
step will be for you to make your donation through the
Independent
Charities of America. Please be sure to carefully follow the
donation instructions provided on the pledge form.
Upon receipt of
your donation, the ICA
will deposit it in the Mother's Day 2005 Project Personal
Charitable Foundation (PCF), which is managed by the Project Steering
Committee.
Upon receipt of your donation, the ICA,
an IRS-recognized 501(c)(3) non-profit organization, will send you
confirmation of your tax-deductible contribution.
The donations deposited in the Mother's
Day 2005 Project PCF can only be disbursed to IRS-recognized
501(c)(3) charities as directed by the Project Steering Committee.
No fees are deducted from your
contribution by the ICA. No member of the Project Steering Committee
will have direct access to the donated funds.
The amount of
you donation will not be publicly disclosed, except at your
request. Your name as a donor will be only be publicly
acknowledged if you so agree on the Pledge Form. A checkbox is
provided for that purpose on the form.

Steering Committee to record and manage the details of your donation, including
tickets purchased. You are welcome to include a dedication in the
space provided on the pledge form.
