How your donation will be managed.

	The first step will be to complete the Pledge Form on this page. The information you provide will ensure proper handling of your donation. The second step will be for you to make your donation through the Independent Charities of America. Please be sure to carefully follow the donation instructions provided on the pledge form.
	Upon receipt of your donation, the ICA will deposit it in the Mother's Day 2005 Project Personal Charitable Foundation (PCF), which is managed by the Project Steering Committee.
	Upon receipt of your donation, the ICA, an IRS-recognized 501(c)(3) non-profit organization, will send you confirmation of your tax-deductible contribution.
	The donations deposited in the Mother's Day 2005 Project PCF can only be disbursed to IRS-recognized 501(c)(3) charities as directed by the Project Steering Committee.
	No fees are deducted from your contribution by the ICA. No member of the Project Steering Committee will have direct access to the donated funds.
	The amount of you donation will not be publicly disclosed, except at your request. Your name as a donor will be only be publicly acknowledged if you so agree on the Pledge Form. A checkbox is provided for that purpose on the form.

Before you donate through the ICA, please complete the Pledge Form below, which will enable the Project Steering Committee to record and manage the details of your donation, including tickets purchased. You are welcome to include a dedication in the space provided on the pledge form.

Donating through the ICA.

	After completing the Pledge form you can make your donation by Check. Secure online credit card payment.
	Donation Instructions: please carefully follow these instructions, which will also be included on the printable "Pledge Confirmation" page, following your completion of the Pledge Form: Check: Make your check in the amount of your total donation payable to: "Independent Charities of America." Please be sure to write "Mother's Day 2005 Project PCF" on your check. Please do note write additional notes on your check. Mail your check to: Independent Charities of America,  2735 E. 53rd Street. Attn: Shannon Smith, Tulsa, OK 74105 Include your contact information, i.e.,  Name, Address, Email, so that the ICA can send you a confirmation of your tax-deductible contribution. Credit card: To donate online via secure credit card transaction, go to: http://www.independentcharities.org Click on the "Give Online" links until you get to the ICA online contribution form. Please be sure to enter "Mother's Day 2005 Project PCF" in the "COMMENT" box on the 3rd page of the ICA online contribution form. Please so not enter additional comments in the "COMMENT BOX." 	Ticket Distribution Tickets ordered on the Pledge Form up to 10 days before the May 8 Concert will be mailed to you. Tickets ordered within 10 days of the May 8 Concert  can be picked up at Temple Shir Tikvah on the day of the Concert.
